DOAC Review SOP

 This standard operating procedure (SOP) outlines a structed medication review for patients on a direct-acting oral anticoagulant (DOAC) for its licensed indication in non-valvular Atrial Fibrillation (AF). Its use is targeted at healthcare professionals in primary care conducting DOAC reviews including but not exclusively pharmacists and pharmacy technicians. The main aim is to facilitate safe and effective ongoing care to patients on DOACs and to ensure they receive appropriate communication and support in taking their medication.
